Howard Dean Formanek, 79, of Chelsea, died Monday, December 30, 2013 at the Marengo Memorial Hospital following a sudden illness. Services will be 11:00 am. Friday at the Hrabak Funeral Home in Belle Plaine by Pastor Holly Oden of Mystic, Iowa. Burial will be in Oak Hill Cemetery, Belle Plaine. Visitation will be from 9:30 am. Friday to service time at the Funeral Home. Howard was born July 25, 1934 in Garrison to Emil and Sylvia Wisner Formanek. He graduated in 1952 from Chelsea High School. He married Lois Burr February 26, 1955 in Belle Plaine. Howard was a Supervisor for the ASCS Office in Vinton for several years. He retired in 1991 and moved to Chelsea. Survivors include sons, Randy and Rick Formanek of Chelsea; Daughter, Roxann Meredith of Mystic , Iowa; sisters, Virginia Upah of Cedar Rapids and Rosemary (Roger) Heller of Marengo; Grandchildren, Misty Meredith, Jessica (Jake) Moorman, Cody (Dannielle) Meredith all of Centerville; Great Grandchildren; Joshua Boddicker of Vinton, Malerie (Thomas) Gage, Zoie Moorman, Katarina, Cyrus Meredith, Bree Boddicker of Tipton. He was preceded in death by his parents; wife Lois in 1968; brother, Richard Formanek. Howard loved hunting, fishing, working with wood, spending time with his family and playing cards with friends.
